Group Adult Foster Care

In-home personal care and more for people age 22 and older who are MassHealth-eligible and live in approved affordable public housing.

This MassHealth-sponsored program is available to residents of approved affordable public housing age 22 and older who need help with one or more activities of daily living.  If you meet these eligibility requirements, we can provide someone to help you in your own home with things like:

  • Getting in and out of bed
  • Bathing
  • Getting dressed
  • Getting around
  • Using the toilet
  • Skin care
  • Medication reminders
  • Meal preparation
  • Companionship
  • Housekeeping
  • Laundry
  • Shopping

Our program gives you lots of choices and options!

  • In-home personal care and other help as many as seven days a week, depending on your needs and preferences.
  • A dedicated case manager and nurse, who alternate visits every 30 days.
  • In-home counseling if you are experiencing mental health issues like depression or anxiety.
  • Referrals to adult day programs for people who want to stay active, build new friendships, and feel secure in a safe environment.
  • Personal emergency response systems.
  • Access to 24-hour assistance.
  • Help arranging and accessing transportation services.

Who is this program for?

This program is for adults living with disabilities age 22 and older who have or are eligible for MassHealth (Medicaid in Massachusetts) and live in approved affordable public housing.

Who benefits from this program?

Your friends and neighbors, people like:

  • Mary, a 53-year-old living with Multiple Sclerosis, who needs assistance with several activities of daily living.
  • June, a 75-year-old widow recovering from a hip fracture, who temporarily needs a little extra help.
  • Dave, a 67-year-old living with COPD, who needs help with personal care, shopping, and housekeeping.
  • Gina, a 24-year-old woman who is blind and recently began living independently in affordable housing for the first time.

Who can refer you?

  • Your physician or specialist
  • A behavioral health specialist
  • Your social worker or case manager
  • A visiting nurse, occupational therapist, or physical therapist before or after discharge
  • Your building manager or resident services coordinator
  • You can refer yourself
